Central Bark Policies and Release of Liability

THIS AGREEMENT is entered into by and between Central Bark Inc. and the dog owner identified below:

1. PHOTO AND VIDEO RELEASE: We love to post pictures and videos on Facebook, our website, and/or in ads. Owner agrees to allow Central Bark to use Owner’s pet’s name and any images or likeness of Owner’s pet taken while he/she is at Central Bark, in any form, for use at any time, in any media, marketing, advertising, illustration, trade or promotional materials without compensation. Owner releases to Central Bark all rights that Owner may possess or claim to such image, likeness, recording, etc.

2. DOG’S HEALTH: Owner further understands and agrees that Owner’s dog(s) are healthy and will at all times while attending Central Bark have current vaccinations as required by Central Bark’s vaccination policy. Owner is not enrolling any dog that has any condition that could potentially jeopardize the health of other dogs or people and has not had any potentially communicable condition within 30 days prior to enrollment. Owner further understands that even if Owner’s dog is vaccinated for Bordetella (Canine Cough), there is a chance that Owner’s dog can still contract Canine Cough. Owner agrees that Owner will not hold Central Bark responsible if Owner’s dog(s) contracts Canine Cough or other dog-to-dog transmitted ailments.

3. FLEA/TICK FREE: All dogs must be flea/tick free. If a dog is observed to have fleas or ticks while in our care, the Owner will be contacted and required to arrange for the dog to be picked up and treated off-site. If the Owner is unavailable to pick up their dog, the designated emergency contact will be required to do so.

4. GROUP PLAY: Owner understands and agrees that in admitting Owner’s dog(s) to dog-to-dog group play, Owner is representing to Central Bark that Owner’s dog(s) is/are in good health and have not harmed, shown aggression, or exhibited any threatening behavior towards any person or other dogs. Any dog demonstrating aggression or other behavior deemed unacceptable by Central Bark staff may be required to leave daycare/boarding or be removed from group play.

5. RELEASE OF LIABILITY: Owner understands and agrees that during normal dog play and while receiving daycare, boarding, grooming, or other services, Owner’s dog may sustain injuries. Dog play and services are monitored/performed by Central Bark staff to best avoid injury, but scratches, punctures, torn ligaments, cuts, nicks, nail quicking, skin irritation, and other injuries may occur despite reasonable care and supervision. Owner further understands and agrees that neither Central Bark nor any of its employees, staff, or volunteers will be liable for any illness, injury, death, and/or escape of Owner’s dog(s), provided that reasonable care and precautions are followed, and Owner hereby releases Central Bark, its staff, and volunteers from liability arising from or as a result of Owner’s dog(s) attending Central Bark.

6. PERSONAL PROPERTY: Owner understands that Owner is solely responsible for any harm, including to any other pet(s), employees or invitees of Central Bark, or to the equipment, facilities, or other property of Central Bark, caused by Owner’s dog(s). Owner also agrees that Central Bark shall not be responsible or liable for any lost or damaged personal property belonging either to Owner or Owner’s pet. Central Bark provides items such as beds, bowls, and other supplies, and recommends that Owners do not bring high-value or irreplaceable personal items.

7. FOOD/MEDS: When boarding overnight, Owner agrees that it is the Owner’s responsibility to leave an adequate supply of food and medications for Owner’s dog(s) during the entire time Owner’s dog is cared for by Central Bark. Should the food supply need replacement, Owner authorizes Central Bark to use discretion in purchasing replacement food or providing food at a cost of $1 per cup.

8. VETERINARIAN LIABILITY AND CARE: Owner authorizes Central Bark staff to provide reasonable first aid when necessary while Owner’s dog is in our care. Owner agrees to allow Central Bark to obtain medical treatment for Owner’s dog(s) if he/she appears ill, injured, or exhibits any other behavior that would reasonably suggest that dog(s) may need medical treatment, including anesthesia. Owner agrees to be fully responsible for the cost of anysuch medical treatment and for the cost of transportation for the purposes of such treatment. Owner gives permission to Central Bark to use Owner’s vet or the nearest appropriate veterinary or 24-hour emergency hospital for required treatment.

10. WEAKENED IMMUNE SYSTEM: Owner understands special-needs dogs, young puppies, and senior dogs naturally have a higher risk of injury, stress-related illnesses, weakened immune systems, or exacerbation of any pre-existing condition. As such, by using our facility for daycare, boarding, or grooming, Owner is accepting the increased risks associated with their dog’s age or condition.

11. ELDERLY DOGS: Owners of elderly dogs (approaching the end of life) need to know that in the event Owner’s dog passes while in our care, our policy is to transport Owner’s dog to Owner’s vet (within 15 miles), where he/she will be held until Owner can be reached by the vet for further instructions. If Owner’s vet is closed or outside of our transport distance, we will transport the dog to the nearest 24-hour facility. Trip charge will apply based on time/distance.

12. GROOMING: Owner understands that although accidents are rare, there is a risk when grooming animals. Grooming equipment is sharp, and although Central Bark uses caution and care, possible problems may include cuts, nicks, scratches, nail quicking, skin irritation, or other injuries, particularly if an animal is moving or wiggling. Central Bark has the right to stop or refuse grooming services if a dog becomes too stressed or dangerous to groom. If a groom cannot be completed, Owner will be responsible for the grooming services performed up until

that point.

13. MATTED COATS: Owner understands that severely matted coats require special attention and that mats can cause or conceal skin irritation, sores, parasites, or other conditions. Central Bark will not cause serious or undue stress to a dog by excessive dematting. Mats may require the dog to be shaved. Removing a heavily matted coat may result in or reveal nicks, cuts, abrasions, skin redness, irritation, hematomas, or other skin conditions. Owner authorizes Central Bark to shave or clip Owner’s dog when reasonably necessary to safely remove severe matting.

14. GROOMING SATISFACTION: Your satisfaction is very important to us. If Owner is unhappy for any reason with a groom, Owner must contact Central Bark within 48 hours. Central Bark will schedule a time for the dog to return for a reasonable adjustment to the original groom at the next available time at no cost.

15. GROOMING CANCELLATION / NO-SHOW POLICY: Grooming appointments cancelled before the day of the scheduled appointment may be cancelled at no charge. The first same-day cancellation or first no-show will be waived as a one-time courtesy. Before future grooming appointments can be booked, Owner must provide a valid credit card to be kept securely on file. Any subsequent same-day cancellation will incur a $25 cancellation fee. Any subsequent no-show will incur a fee equal to 50% of the scheduled grooming service. Applicable fees may be charged to the credit card on file and outstanding fees must be paid before future services will be  provided.

16. PREPAID PACKAGES: Owner understands that prepaid daycare packages are nonrefundable and expire 12 months from date of purchase. Discounted packages are provided only if they are paid for in advance.

17. PAYMENTS: Payments are due at the end of each visit. Boarding stays estimated at $750 or more may require partial or full payment at check-in, with any remaining balance due at pickup. Payment may be made by cash, Visa, Debit, or Mastercard.

Central Bark also reserves the right to stop or refuse any service if staff determine it cannot be safely provided. The determination shall be made at the sole discretion of Central Bark.

21. FULL FORCE AND EFFECT: Owner further understands and expressly agrees that each and every of the foregoing provisions contained in paragraphs 1-20 shall be in force and effect and shall apply to each and every occasion on which Owner’s dog(s) attends Central Bark for daycare, boarding, grooming, or other services, as the case may be.This Agreement shall remain in full force and effect between the parties until and unless otherwise cancelled or superseded by a writing signed by the parties.
